ABSTRACT

Introduction: In the pandemic, porlonged weaning(PW) of mechanical ventilation (wMV) of COVID patients in the intermediate respiratory care unit IRCU was performed. It was necessary to use predictive indexes for(wMV), which do not generate aerosolization of viral particles. Objetive: To develop a oredictive indexes for wMV and tracheostomy decannulation (TCHd) for COVID-19 pneumonia in IRCU. Method(s): The sample consists of 76 serial cases to the IRCU, in 2020 and 2021. Indexes were developed with an oxygenation variable (PaO2/FiO2) or (SatO2/FiO2), respiratory rate (RR) and corrected (C) in based alveolar ventilation (PCO2), the following indexes were developed as predictors of wMV;ventilation-oxygenation index IVOX= (PaFi/RR), IVOX corrected for PCO2 is IVOX-C=(IVOX x Oco2) and with SaFi the SIVOX-C= [(SaFi/RR) x PCO2]. The StatPlus 7.3 program forWindows was used of the Mann-Whitney U (M-WU) comparing their mean values, using binary logistic regression (BLR) and area under curve AUC ROC to compare their predictability. Result(s): Mean age 58,9 +/-14,4;male 53,7% and the stay in the IRCU was 16,7+/- 11 days, mortality of 28,3%(22);received MV (71,0%) 54. wMV was(70,4%)38 and TCHd was (67,3)35. The mean differrences in disconnected and non-diconnected from MV analyzed by M-WU are significante. An BLR model was built to analyze the predictive behavior ofIVOX, IVOX-C and SIVOX-C for wMV. It was observed that the three indexes are predictive, but IVOX-C and SIVOX-C have the highest predictive weight. In turn the AUC ROC was significance. Conclusion(s): The construction of a predictive indexes of wMV and TCHd in this sample the patients who reached the objective.

ABSTRACT

Background: The Coronavirus-2 (SARS-CoV-2) pandemic has generated negative psychological effects in the whole population. Our objective was to describe this impact on health care workers of Argentina. Methods: This was a cross-sectional, multicenter study evaluating anxiety, depression, burnout, and alterations in lifestyle and interpersonal relationships by means of an anonymous survey carried out from June to September 2020. Results: A total of 1221 healthcare workers, with median age 42 years (IQR 34.5-52) and 65% women were surveyed for the study. Among them, 77.2% lived in the Autonomous City of Buenos Aires and the Buenos Aires province;66.7% were specialist physicians or nurses, 41.1% were clinical specialists, 29.8% intensive care or coronary care unit physicians and 10.2% kinesiologists or nurses. In 51.8% of cases, healthcare workers reported that all biosafety recommendations were not met in their workplaces. Anxiety was detected in 40.5% of cases, major depression in 22.1% (4.5% referred frequent suicidal ideation), minor depression in10.9% and burnout in 38.7%. Smoking increased in 9.4% of healthcare workers and alcohol consumption in 22%;62.1% declared worse sleep quality, 50.2% reduced physical activity, 18.4% worsened their affective relationship and 40.4% suffered discrimination for being health workers. Conclusion: The Coronavirus pandemic has produced an increase in depression, anxiety, burnout and unhealthy lifestyle habits in healthcare workers surveyed in Argentina.

ABSTRACT

Background: The COVID-19 pandemic led to the implementation of massive lockdown measures producing a reduction in consultations, diagnostic tests, treatments and admissions for cardiovascular and cerebrovascular events and cancer. Objective: The aim of this study was to determine the access to medical care and health care information in patients with cardiovascular diseases during social, preventive, and mandatory confinement in Argentina. Results: A total of 1,487 surveys were analyzed. Mean age was 56.17 ± 14.0 years;66.8% of survey respondents were women, 38.9% lived in the Greater Buenos Aires area and 27.1 % in the Autonomous City of Buenos Aires. The most commonly reported main cardiovascular disease was hypertension (52.1%);42% required medical care and 57.9% could not access to consultation due to issues related with the health system in 68.2% of cases. Only 16.4% did not request medical care for fear of becoming infected with COVID-19. Seventy-two percent required prescriptions and 13% did not obtain them. Access to vaccination was normal in 41.4% of respondents. Social mandatory isolation was respected in 92.4% of cases. Finally, 54.6% of the surveyed population felt unprotected by the health system. Conclusions: We believe that access of the population with chronic cardiovascular diseases to the health system should be warranted in order to adapt monitoring and minimize hospitalization due to decompensation in this high-risk population.
